Community nurses are based in most primary schools and early childhood centres. Jasmine Cottage provides support, counselling, education, and
groups for families with children under 5 years.
Community Nurse Referrals
A registered nurse is available, for short periods of time weekdays (and sometimes on
weekends for essential services only), to assist with medication monitoring, injections,
bathing, wound dressing, and advice on nursing management. Community nurses (now called
Primary Health Nurses) are often asked to follow up patients discharged from hospital to
continue a specified treatment or monitor the patients overall care. Referral can be made
by anyone. Nurses do an initial assessment to determine needs. This service is free.

Aged Care Psychogeriatric Community Nurse
Provides assessment, advice, counselling and case management for elderly people who
develop psychiatric illness and/or dementias. Client/Carer Support and education is a
feature of the service. The position liaises closely with the community mental health
teams, ACAT and inpatient facilities.

Aged Care Nurse
The Aged Care Nurses are part of the Aged Care Assessment Team. They conduct home
visits or telephone consultations providing assessments, information, advice and
assistance to older people most appropriate to meet their needs. Services may be organised
to assist persons to remain in their homes or entry facilitated into an appropriate
nursing residential aged care facility.

Consist of an inpatient unit at Camden Hospital of 10 beds, a staff specialst, a hospital
consultative service, outpatient clinics at Camden PCU and Macarthur Cancer Therapy
Centre, home visits and nursing home visits.
Nursing Services
Consist of the Palliative Care Community Nursing Service (offering home visits, nursing
home visits, a hospital consultative service and out patient clinics), primary health
nurses, and an after hours call centre.

Palliative Care involves the whollistic care of clients and their carers who are
experiencing a life threatening illness, such as cancer, HIV/AIDS, chronic cardiac, lung,
liver, kidney diseases, multiple sclerosis and motor neurone disesase.
The service offers education, information, consultancy and support to general
practitioners, specialists, nurses and other health professionals who care for persons,
particularly regarding symptom control, and other physical and emotional needs of the
clients and their significant others.
An after hours/weekend service may be available to clients registered with the service.

Palliative Care Association of NSW
Palliative care is specialised health care of dying people which aims to maximise
quality of life and assist families and carers during and after death. Palliative care can
provide:
- medical, nursing and other health care
- Equipment to facilitate care
- Temporary respite care to give carers time out
- Volunteers who provide a range of services to patients, carers and families
- Counselling for patients and families
- Support groups for patients, carers and families
- Palliative acre recognises that dying people from different indigenous and non-English
speaking backgrounds have special needs.
- Palliative care can support culturally diverse customs, traditions, religious beliefs
and practices.
