SSWGPL’s Dietitians and Exercise Physiologists can now provide services in your practice.
The Chronic Disease Management and Prveention (CDMP) Program has restructured service delivery to be able to provide access to a Dietitian/Exercise Physiologist in your practice on a regular basis. We recognise that access to the CDMP allied health locations can be a barrier to both GPs and their patients.
Over the next 12 months SSWGPL is rolling out the option of having a CDMP staff member in your practice providing individual consultations at no cost to you. All you will need to provide is a consultation room and referrals. CDMP staff are covered for indemnity insurance as employees of SSWGPL.
